Participation Risks & Acknowledgments:

1. 1. Voluntary Participation: I am voluntarily participating in this food challenge and am doing so at my own risk.
2. 2. Health & Safety Warning: I understand that consuming large quantities of food quickly can be dangerous and may lead to choking, nausea, vomiting, allergic reactions, or other medical complications. I confirm I am in good health and have no medical conditions (including but not limited to gastrointestinal, heart, or eating disorders) that could be affected by this challenge.
3. 3. No Medical Assistance Provided: I understand that Johnny’s/Johnny Brusco’s New York Style Pizza does not provide on-site medical assistance or supervision. In the case of an emergency, emergency services (911) will be contacted. Johnny’s/Johnny Brusco’s New York Style Pizza is not responsible for any expense associated with calling 911, the medical services provided or any needed medical transportation provided
4. 4. Assumption of Risk: I fully assume all risks, known and unknown, related to my participation, including the risk of serious injury or death.
5. 5. Release of Liability: I hereby release and hold harmless Johnny’s/Johnny Brusco’s New York Style Pizza, its owners, employees, affiliates, and representatives from any liability, claims, or actions resulting from my participation in this challenge.
6. 6. Media Release: I grant permission to Johnny’s/Johnny Brusco’s New York Style Pizza to use my name, image, and any photos/videos taken during the challenge for promotional, advertising, or marketing purposes without compensation.
